Current faculty, staff, and students may use this form to submit recognitions for current faculty and staff. Each submission will be carefully reviewed and may be summarized before distribution to the employee and any internal platforms. Further, some submissions may be distributed only to the employee and their supervisor rather than listed on applicable internal platforms. IUSB HR reserves the right to determine how each submission will be shared.
